2009 Amanda Haxton Memorial Scholarship
The Amanda Haxton Memorial Scholarship will be awarded to a 2010 high
school graduate and Delaware Shore Field Hockey participant who plans to
attend a 4-year college or university or a medical training facility or
program.
Applicants must meet the following four (4) criteria:
1. The Haxton Scholarship will be awarded to a graduate of the Class of 2010, pending
proof of
a. High school graduation (2010)
b. Acceptance into a four-year college/university or a medical training facility
or program.
2. The applicant must submit a cover letter that includes
a. Name, mailing address, and e-mail address
b. List of involvement in Delaware Shore Field Hockey
c. List of participation in high school and other field hockey activities
3. The applicant must submit an anonymous letter or essay that includes
a. How field hockey has impacted her life so far
b. The role of field hockey in her future plans
c. Financial need for college
4. Applications must be electronically submitted to HaxtonApp@delawareshorefh.org on
or before December 4, 2009. Applicants will receive notification by electronic
mail from the Executive Director of Delaware Shore Field Hockey, Inc.
Funding for the Haxton Scholarship will be provided partially from the proceeds of the Haxton & Easter Seals Tournament,
December 19-20, 2009, and other fundraisers during the tournament.
Special consideration will be given to applicants who plan to major in the field of health or medicine.
